Referrals

There are several ways to obtain a referral to attend the day care service at Seagrove. We have worked to make this process as simple and straightforward as possible, and full details are presented below.

If you would like to discuss a referral, please get in touch.

Qualifying for a partially funded referral

Partially funded places are available for our day care service, with the Local Authority covering most of the costs. Members are required to contribute £11.03 per day to the cost of providing transport with an escort, breakfast on arrival, activities and day trips, and a two-course hot lunch.

To qualify for a funded place, the individual must:

  • Live in one of the following postcode areas: EH6, EH7, and parts of EH5 and EH8*

  • Be over the age of 65 (or under the age of 65 but with an official dementia diagnosis)

  • Have an official diagnosis of dementia either from their GP, their consultant or through attendance at a memory clinic

  • Be Frail/Elderly and meet the criteria set out by Edinburgh Health & Social Care Partnership

*If the individual lives outside of Seagrove’s catchment area, their application make still be considered, but we would be unable to provide transport to and from the centre. We do have members from other areas who have their own transport arrangements and regularly attend, so do feel free to get in touch to discuss this.

Individuals should also:

  • be referred by Social Work, Occupational Therapy, the Community Mental Health Team, a Community Psychiatric Nurse, or Dementia Link workers.

OR:

  • be in receipt of a direct payment in respect of their day care.

Private or self-funded referrals

We also offer individuals the option to pay for their own space at Seagrove through a private referral. This allows those who fall outside of the criteria listed above to still benefit from our services. It also means that members can book additional days with us on top of those covered by their Local Authority allocation.

The cost for self-funded or private referrals is £75.50 per day.

Taster sessions

Prospective members may come along to Seagrove for a tester session, which allows them to meet our team and experience our services firsthand.
